The location of the lesion is important to obtain adequate tissue. Sampling of periesophageal lesions, such as mediastinal lymph nodes, is relatively easy because they are closer to the scope and the endoscopist can obtain tissue while maintaining a straight, stable scope position. This may apply to submucosal lesions of the gastrointestinal tract. A retrospective study evaluating the diagnostic yield of submucosal lesions reported an 89% diagnostic yield with EUS-guided sampling. In contrast, the sampling of lesions in the uncinate process and pancreatic head is relatively difficult because the lesions are far away from the lens, which is relatively unstable in the anterior half of the duodenum. The study reported an 89% diagnostic rate for EUS-guided sampling.
