1. Patella cartilage cutting: including superficial cutting of cartilage, cutting of cartilage to bone and bone drilling.

① Superficial cutting of cartilage:

The degraded cartilage is cut with a sharp knife until it is normal. Although the repair ability of the cartilage was weak after superficial cutting, the surface became smooth and covered with several layers of flat cells after several months of shaping.

② Cartilage cutting to bone:

If cartilage damage has reached bone, the whole layer of cartilage can be cut, the edge of the wound can be trimmed to make an inclined plane, and the exposed bone is not treated. Slow endogenous regeneration can be obtained from full-thickness cartilage defects without reaching the medullary cavity, and the regenerated cartilage is hyaline cartilage.

③ Cartilage cutting to bone and drilling:

The whole layer of diseased cartilage was cut, the exposed bone was drilled with several holes with Kirschner needle to cause bone bed bleeding, and the whole layer of articular cartilage defect deep into the medullary cavity could be repaired by exogenous mesenchymal tissue from the medullary cavity.

The above operations can be completed by arthroscopy, cutting with a planer knife, or arthrotomy under direct vision.
