Beyond delivery, microneedles are pivotal in diagnostics and monitoring. Traditional phlebotomy requires venipuncture, specialized equipment, and trained staff. Microneedles reduce fluid sampling to a Band-Aid-like application, enabling home healthcare and telemedicine.
Microneedles enable painless blood/interstitial fluid (ISF) collection. Arrays penetrate to the papillary dermis, accessing capillaries and ISF. Integrated microfluidic channels or absorbent pads harvest microliter-scale samples. This is invaluable for chronic disease monitoring; diabetic patients can use patches to collect ISF for external glucose sensing, avoiding daily fingersticks.
Further, microneedles integrate with biosensors for real-time continuous monitoring. "Smart patches" embed glucose oxidase electrodes, pH sensors, or lactate detectors. Upon insertion, sensors interface with ISF, converting biochemical signals to electrical outputs transmitted via Bluetooth to smartphones. Wearable systems allow patients to track glucose, lactate, or electrolytes continuously, facilitating timely interventions. Athletes use lactate-monitoring patches to optimize training intensity based on muscle fatigue.
In early disease screening, microneedles detect tumor markers, cytokines, or drug levels via ISF sampling. ISF biomarkers correlate strongly with blood levels, and sampling is painless and minimally invasive-ideal for mass screening. A prostate-specific antigen (PSA) detection patch delivers results in 15 minutes with >95% sensitivity, aiding early prostate cancer detection.
Finally, microneedles assist therapeutic drug monitoring. Patients on narrow-therapeutic-index drugs (warfarin, cyclosporine) require frequent blood tests. Microneedle patches can hourly sample ISF and analyze drug concentrations, enabling precise dose adjustments to avoid toxicity or inefficacy. Microneedling is effectively condensing the "hospital laboratory" into a discreet skin-adherent patch.
