For FNA, a 22-gauge x 1.5-inch needle is sufficient to obtain cytology samples for most lesions; however, deeper lesions may require longer spinal needles. The stylet used with the spinal needle reduces the chance of collecting non-diagnostic tissue (such as abdominal fat) during placement. The stylet stays in place until it reaches the lesion. A smaller 25-gauge needle can be used to sample lesions that are prone to blood contamination.
The core biopsy needles vary in size from 14 to 18 and vary in length. Both automatic and semi-automatic core biopsy systems are available. Automated systems may be more economical and more accurate, but because they eject the needle tip beyond the needle placement point, a full assessment of the depth of the lesion is required. A fully automated biopsy needle can cause a fatal shock response, so it is best to avoid using it on the body. The advantage of semi-automatic systems is that they will not eject the cutting core beyond the needle tip visible in the lesion. This may help prevent inadvertent penetration of surrounding tissues.
