PS Tip Runs The Whole Room

The Pain: Tip Drawer Chaos

Hygienists open a tray with 14 tips: G1, G2, P1, P3, P4, PS, PSL, PSR, PI, E1, E2, diamond furcation tip, stain tip, implant plastic tip. Everyone uses a different one. New grads grab G2 for everything. Experienced perio nurses refuse G-tips entirely. The patient gets a different "cleaning philosophy" every visit. Outcomes cannot be audited because the instrument sequence is never the same.

This is not a staff problem. It is a protocol problem. Too many tips create freedom without competence.

The Principle: One Slim Tip, Controlled Motion

EMS PS Instrument is intentionally boring-in the best way. Slim like a periodontal probe, smooth sides, linear piezo motion, supragingival to 10 mm subgingival. Because the tip is narrow, lateral aggression is low. Because piezo motion is linear, the working side follows the root like a wiper.

Used at 30–40% power, PS often outperforms a G2 hammered at 80% power. Lower power means less lateral pressure, less cementum removal, less patient flinch. EMS's own guidance says PS covers about 95% of cases; PSL/PSR/P exist for the remaining 5%: heavy interproximal calculus, stubborn concrements, curved access.

Equipment Classification

  • PS: default adult prophy + perio maintenance.
  • P: hard calculus up to 4 mm, initial therapy.
  • PSL / PSR: heavy calculus, mesial/distal contacts, up to 8 mm, adults only.
  • PI Max: implant/veneer/crown/bracket maintenance up to 3 mm.
  • G-series: traditional supragingival work if practice prefers wider tips.
  • E-series: endo only, never hygiene.

Practical Guide

New adult recall: PS, low power, medium-high water.

Heavy smoker stain: PS first; escalate to G2/G6 or P if deposit resists.

Pocket 4–6 mm: PS side-adapted, short oblique strokes.

Pocket 7–10 mm mandibular molar: PS, then PSL/PSR if access fails.

Implant: PI Max only.

Ortho bracket: PI Max or plastic tip.

Pediatric primary teeth: PS or PI Max, low power.

Replace PS when 2 mm wear occurs or check-card says fail.

Real-World Experience

A Scandinavian GBT practice forced "PS first, everything else is escalation." Within two quarters:

bleeding on probing dropped,

recall compliance improved,

patients described cleanings as "different-no scraping feeling,"

hygienists stopped arguing about tip kits.

The breakthrough was not buying more tips. It was deleting decision fatigue.

Summary

PS is not a tip. It is a clinical standard. Fewer instruments, better angulation, repeatable outcomes.

Outlook & Suggestions

Future software may lock power bands to tip ID. Today, build a PS-first SOP, keep P/PSL/PI as exception tips, and train every new hire on why PS exists before they ever touch G2.
