The indications, technique, and results of trocar nephrostomy are performed by percutaneously puncturing the renal pelvis with a large needle, and then inserting a plastic tube for temporary urination in selected cases of hydronephrosis. The pyelogram is made by lumbar puncture of large hydronephrosis, then aspiration of urine and injection of urinary tract contrast agent. Trocar nephrostomy is a natural product of antegrade pyelography. Technique The lumbar puncture of hydronephrosis and the insertion of the plastic tube are controlled by radiography under local anesthesia. Follow the same precautions as when performing a spinal tap. The patient lies on his stomach. The preliminary X-rays locate the approximate puncture points associated with the lumbar spine and vertebral markers.
