Based mainly on clinical evidence, it has been suggested that the dural injury caused by pencil tip needles is minor. The concept of dural lesions associated with Quincke-type needles developed in the 1940s May no longer be valid, 1™ because of the lack of adequate bevelled edges in older needles, producing larger lesions more by tearing action than by sectionizing. Modern needles, however, produce clean U-shaped lesions or flaps, similar to the open lid of a tin can, and form through the superposition of damaged flakes. When the needle is withdrawn, the U-shaped flap tends to revert to its original position, favored by CSF pressure and the viscoelasticity of the dura, resulting in almost complete obstruction of the dura foramen after 15 minutes. On the other hand, the lesions produced by pencil tip needles are not only consistent with the dural fiber separation theory, but may also involve complex fiber tearing, sectioning, and separation mechanisms, in which needle design may be critical.
