Postdural headache (PDPH) is a recognized complication of spinal and epidural anesthesia. It may also occur after a diagnostic or therapeutic lumbar puncture. Few cases have been reported in children. We reviewed the literature on definition, etiology, incidence, risk factors, prevention, and treatment to provide some recommendations. Important factors included age, sex, needle diameter, needle tip design, needle tip orientation at the time of puncture, previous PDPH, migraine history, and repeated puncture attempts. There was no evidence of increased fluid use or bed rest to prevent PDPH. Once a diagnosis is made, conservative treatment for 48 hours is recommended. Persistent PDPH can be treated in a variety of ways; Epidural blood patches are one of the most effective methods. Conclusion: The incidence of postdural headache after lumbar puncture in children is relatively rare. There are conflicting data on risk factors, prevention and treatment.
