B ultrasound guided percutaneous transhepatic gallbladder puncture catheter drainage. The specific method was as following: the patient was in supine position, and the skin of the seventh and eighth intercostal Spaces of the right midclavicular line was disinfected under the guidance of B-ultrasound. After local anesthesia, a 0.13cm incision was made in the skin, and PTCD needle was inserted into the direction of the gallbladder through the liver tissue and gallbladder bed, and breakthrough feeling was felt when penetrating into the gallbladder. The purulent bile can be extracted by pulling out the needle core, and some of it can be taken for bacterial culture. Then, the guide wire is inserted into the gall bladder through the needle tube and indwelled, the needle tube is pulled out, and the pig tail tube is inserted into the guide wire, and the tube is inserted into the gall bladder 4~6cm along the guide wire. Pull out the guide wire, fix the catheter to the skin, and connect the sterile bag. If bile is sticky, usable normal saline or metronidazole infuses gallbladder, dilute bile can lead out. In this study, PTGBD gallbladder puncture and catheter drainage were all successful, with a maximum of 160ml and a minimum of 30ml pus drainage. Bile was routinely examined and cultured, and the drainage tube was fixed.
