How does clavicle fracture anaesthesia locate

According to different puncture sites, it can be divided into the following three methods:

① Muscular sulcus method: the preferred anesthesia method for shoulder and upper arm surgery. Patients go to occipital recumbent, head to the opposite side, the affected side shoulder cushion under the thin pillow, the upper limbs close to the side. The posterior edge of the sternocleidomastoid muscle above the clavicle touched a triangular gap formed by the anterior and middle scalenus muscle and omohyoid muscle. The subclavian artery could be touched at the bottom of the triangle to pulsate, and the puncture point was equivalent to the level of the sixth cervical vertebra at the edge of the cricoid cartilage.

② Axillary method: it is suitable for the operation of the lower 1/3 part of the upper arm or the surgical reduction of fracture, and the ulnar part of the hand, wrist and forearm is the first choice. The patient was supine and pillow removed, the affected limbs were extended 90 degrees, the flexed limbs were 90 degrees, the back of the hand was close to the bed and the head was in a military salute, and the armpit was completely exposed, and the axillary artery pulsation was felt in the armpit, and the highest point of the artery pulsation was the puncture point.

(3) Supraclavicular method: the patient lies supine, pads a thin pillow under the affected shoulder, turns the head to the opposite side, and routinely disinfect the skin with a towel. A local anaesthetic was used to make the skin mound at about 1cm above the midpoint of the clavicle. A 6 1/2 3.5cm injection needle was inserted inward, backward and downward to find the first rib. The needle was inserted 1-3cm to Pierce the rib and abnormal sensation was found along the rib. Fan-shaped injection can be performed along the ribs in case of abnormal sensation.
