Can bone marrow puncture damage the body?
Aug 22, 2022
Bone marrow aspiration, referred to as bone piercing, is one of the most commonly chosen tests by hematologists. When patients present with multilineage blood cell abnormalities or anemia, leukopenia and thrombocytopenia that cannot be explained by general reasons, bone perforation examination is necessary to understand the proliferation of bone marrow and the presence of abnormal cell infiltration. Due to the influence of traditional concepts, some patients will be afraid when they hear about bone piercing examination, worried that bone piercing will affect the body. In fact, this fear is unnecessary. Bone marrow puncture examination, just like blood drawing examination, only needs to draw a small amount of bone marrow fluid to meet the needs of examination, and the lost bone marrow fluid will soon be replaced by hematopoietic cells in the body, without any adverse effects on the body. Bone marrow aspiration is also necessary for the diagnosis of leukemia, multiple myeloma and other hematological malignancies. Part of bone marrow fluid is usually retained for chromosome and genetic examination, which can help the diagnosis and analysis of prognostic risk factors. Reexamination of bone perforation after treatment can evaluate the therapeutic effect. Some understanding of the process of bone penetration can help patients eliminate unnecessary worries and actively cooperate with the examination. The common puncture sites are the anterior superior iliac spine, posterior superior iliac spine and sternum. The anterior superior iliac spine and posterior superior iliac spine are the areas where the sides of the pelvis edge protrudes outward. Because these two areas are close to the skin surface, the bone surface is wide and flat, and the pulp content is rich, they are often selected as puncture sites. The sternum is also wide, flat, and close to the body surface. However, due to the mediastinum and the lung and heart in the chest cavity on both sides and behind it, sternal puncture is rarely selected, and attention should be paid to prevent injury during operation. Before bone perforation, local anesthesia is used. A piculum is injected with anesthesia, and then the injection needle is inserted subcutaneously until the periosteum, on which the anesthetic is injected. The quality of periosteal anesthesia is the key to puncture pain. After the needle enters the bone marrow cavity, a syringe is attached to aspirate the bone marrow. When the marrow is withdrawn, the patient may experience a momentary sensation of soreness. After the bone marrow smear is completed, clean gauze and tape are applied to the bone piercing site. After a short rest, normal activities can be performed. Note that the puncture site should not be soaked with water within 3 days. After some patients bone is worn, there are still two or three days to feel local acid distension, this is a normal phenomenon, need not worry too much, the symptom will get better very quickly.
