The Most Overlooked Critical Step in Vacuum-Assisted Breast Biopsy

Vacuum-Assisted Biopsy (VAB) is the gold standard for large-volume sampling and microcalcification retrieval. Unlike manual spring guns, VAB systems integrate precision motors, vacuum lines, and sensors. Here, "reset" bifurcates into System Initialization (Homing) upon probe installation and Automatic Recocking between samples, plus a mandatory Pre-Withdrawal Retract/Home command. These are frequently underestimated yet are common sources of device error or patient injury.

1. System Initialization (Homing / Zeroing) After Probe Mounting

Once the VAB probe is locked into the driver handle:

  • Action: Press the "Sample" or foot pedal onceto trigger initialization.
  • Mechanism: The internal motor spins the rotary cutter, drives it through the probe's sampling window, then precisely retracts it to the Home Position (cutter fully covers the window).

Purpose:

  • Calibrates absolute cutter positionfor accurate forward-advance and retreat cycles.
  • Closes the sampling window-a vital safety state preventing the open slot from hooking normal tissue during insertion.
  • Checks vacuum seal(on some platforms).

Abnormality: Error codes (e.g., E-01) indicate incomplete homing-probe not fully seated, drive jammed, or expired probe. Never insert the probe if homing fails.

2. Inter-Sample Reset: Recocking (Automatic)

Pressing "Sample" during surgery executes: (a) Cutter advances & severs tissue drawn into the vacuum slot, then (b) automatically retracts (Recocks) to the home/closed-window position. This auto-reset ensures the window is shielded during probe rotation or depth adjustment between passes.

3. Pre-Withdrawal Reset: Retract / Home Key

At procedure end, before removing the probe:

  • Risk: If the cutter is forward or the window is open, the exposed rotating element or sharp slot can severely lacerate breast parenchyma, fat, or pectoralis major during withdrawal → hemorrhage/hematoma.
  • Correct Action: Press "Retract" or "Home." Confirm on-screen icon/text that the cutter is fully retracted inside the protective tip. Visually verify if possible. Thenwithdraw slowly.

4. Clinical SOP Recommendations

  • "Install → Home → Insert" Rule: Probe installed → Press Sample for Homing → Confirm "Ready"/Window Closed → Begin insertion.
  • "Open Slot Mode" Caution: If using manual open-slot positioning, alwayspress "Close" to reset/retract before any probe advancement or removal.
  • Shift-Change Verification: Incoming staff must confirm device displays "Home" status.
  • Treat Homing and Recocking as the VAB procedure's "seatbelt." Cultivate the habit: No Home = No Insert. No Home = No Withdraw.
