The high diagnostic accuracy of Core Needle Breast Biopsy (CNB) relies heavily on precise image guidance and standardized protocols. Ultrasound-Guided CNB (US-CNB) is most prevalent, while Stereotactic CNB (SCNB) targets microcalcifications visible only on mammography, and MRI guidance serves select cases.
1. Key Steps for Ultrasound-Guided CNB (US-CNB)
- Positioning & Probe: Patient supine or lateral decubitus with ipsilateral arm abducted. High-frequency linear probe (7.5–12 MHz) confirms lesion size, margins, vascularity, and distance from skin/chest wall.
- Trajectory Planning: Select the shortest path avoiding major vessels and lactiferous ducts. Shallow lesions require shallow angles to prevent traversing the breast. Deep lesions near the chest wall require vigilance to avoid pleural puncture (extremely rare).
- Local Anesthesia: 1% Lidocaine infiltrated along the anticipated track to the lesion's superficial aspect. Avoid injecting directly into the lesion, which causes edema and distorts sampling.
- Firing & Sampling: Needle tip advanced to lesion edge → Inner stylet retracted exposing the notch → Confirm notch within target → Fire. Typically obtain 3–6 cores (≥4 cores recommended for malignancy/molecular profiling, sampling different quadrants). Avoid necrotic centers with initial/final passes.
- Coaxial Technique: A 14G or 18G coaxial introducer is placed first. The sampling stylet is passed through it multiple times, minimizing skin punctures, reducing hematoma risk, and allowing clip marker deployment via the sheath post-sampling for surgical/radiotherapy localization.
2. Stereotactic CNB (SCNB)
Indicated for BI-RADS 4/5 lesions presenting solely as clustered microcalcifications. The patient is prone or upright with breast compression. Biplane imaging calculates 3D coordinates. The needle is advanced automatically or manually to the target for firing. Due to calcification friability, obtain ≥5 cores and perform specimen radiography to confirm calcification retrieval before sending to pathology.
3. Sampling Frequency and False Negative Mitigation
Literature cites CNB false negative rates of ~2–5%, primarily due to sampling error ("blind men and the elephant" effect):
Sample multiple sectors for multifocal lesions or tumors >3cm.
If imaging-pathology discordance exists (high clinical suspicion but benign CNB), repeat CNB or proceed to excisional biopsy.
14G sensitivity for non-mass lesions <10mm is slightly lower than 16G with multiple passes; consider upgrading to Vacuum-Assisted Biopsy (VAB) if needed.
