In cases where gas or bone prevents entry into the lesion, ultrasound-guided needle biopsy may not be possible. Repositioning the patient or changing the position of the transducer may produce a better window. Otherwise, computed tomography, fluoroscopy, or open surgical sampling may be required. Structures and lesions located deep in the pelvis may not be accessible through the abdominal approach. In cases involving the prostate, bladder, and distal urethra, ultrasound guidance can still be used to obtain samples through traumatic catheterization.
Although some larger gastrointestinal masses may be suitable for ultrasound-guided biopsy, smaller lesions may be more difficult to enter and peel off, especially if they avoid penetrating the gastrointestinal lumen. Laparoscopy or surgical biopsy is suitable for these cases.
