How relevant is the Franseen biopsy needle? Why is it so important? 

One, the ability to obtain a sufficient volume of tissue using the Franseen biopsy needle makes the performance of ancillary studies reliable. Therefore, EUS-guided tissue collection with the FNB needle may be specifically requested when a rare diagnosis is suspected or when ancillary work is required. Second, the diagnostic sensitivity of EUS depends on the ability of the cytopathologist to provide rapid on-site assessment. This restriction limits the use of EUS to those that are most readily available. A common alternative technique is to prepare a unit block for off-site evaluation. However, the diagnostic yield and accuracy of cell clusters using standard FNA is only 75%–80%. This is because aspirates often contain a limited number of cells and lack the pro-fibrotic matrix exfoliated by the glands, both of which are histological hallmarks of malignancy. Franseen biopsy needles are able to obtain large numbers of tumor cells and interstitial fibrosis, resulting in better histological samples, allowing pathologists to diagnose tumors with relative ease. This is evident in the high diagnostic adequacy (>95%) and accuracy (>90%) of cell clumps biopsied using Franseen.
