When mammogram reports indicate "clustered microcalcifications" or "structural distortion," patients often experience anxiety: Could this be cancer? At this time, stereotactic breast biopsy becomes a crucial bridge connecting imaging abnormalities and pathological diagnosis.

I. Typical Indications

Stereotactic biopsy is not a "one-size-fits-all" solution. Its optimal applications include:

  • 1. Assessment of microcalcifications: BI-RADS 4–5 calcifications, especially clustered, linear, or branching calcifications;
  • 2. Non-palpable masses: not palpable on physical examination but visible on imaging;
  • 3. Structural distortions: new distortions after excluding postoperative scarring;
  • 4. Follow-up for previous false negatives: previously negative biopsies but clinically highly suspected malignancy.

II. The Role of Biopsy Needles in Clinical Decision-Making

The choice of biopsy needle directly affects diagnostic efficiency in different clinical scenarios:

- Initial screening for calcifications: 14G stainless steel hollow needles are recommended, balancing sample volume and operational costs;

- Re-biopsy or high-risk lesions: 11G vacuum-assisted biopsy needles can be used, allowing for the collection of several centimeters of tissue strips in a single procedure;

III. Standard Operating Procedure Analysis

  • 1. Patient Positioning: The patient lies prone on a specialized biopsy table, with the breast naturally drooping through the compression window.
  • 2. Image Localization: Stereoscopic images are acquired, and the center of the lesion is marked.
  • 3. Disinfection and Anesthesia: Local infiltration anesthesia is administered, taking care to avoid nerves in the areola area.
  • 4. Puncture and Sampling: The biopsy needle is inserted at a predetermined angle, typically yielding 3–6 tissue samples.
  • 5. Specimen Processing: X-ray imaging confirms the presence of calcifications, followed by pathological examination.

IV. Complication Management and Patient Communication

The most common concerns in clinical practice are bleeding and pain. In fact, the standardized use of a blunt-tipped biopsy needle with side holes can effectively reduce vascular injury. After 24 hours of pressure bandaging post-procedure, over 90% of hematomas will resolve spontaneously. V. The Closed Loop from Diagnosis to Treatment

- Carcinoma in situ: Breast-conserving surgery + radiotherapy may be considered;

- Invasive carcinoma: Further molecular subtyping evaluation is required.
