In the out-of-plane method, the needle passes perpendicular to the transducer, so the needle passes through the sound beam plane image. The disadvantage of this method is that only a small section of the needle can be seen where the needle passes through the plane of the sound beam. When using the out-of-plane method, the needle insertion site is located at the center above the target, and the needle insertion enters from the side of the transducer. When inserting the needle, some trial and error is required as the needle passes through the acoustic beam, retracts, and then repeatedly redirects (usually deeper) to finally reach the target. Another disadvantage is that it is impossible to determine which part of the needle, the shaft or tip of the needle is shown on the ultrasound image, because they look exactly like a single bright echo with reverberation artifacts. Although the in-plane method is preferred in most cases, the out-of-plane method is more popular in situations where the target is very shallow, such as guiding the needle into the small joints of the hands and feet. Regardless of the in-plane method or the out-of-plane method, once the needle is seen in the target, the sensor must be rotated 90 degrees to confirm the exact position of the needle tip. It is wise to adapt the direct method of needle guidance both in-plane and out-of-plane.
