What should I do after particle implantation?

Jun 21, 2022

The first day after radioactive particle implantation, the anteroposterial-lateral chest radiographs were examined. Chest CT was reviewed at 1, 3, 6, 9 and 12 months postoperatively. A complete examination was performed at 6 and 12 months, with the same items as before. PETCT or SPETCT is recommended to detect and record metastatic cancer; In the second year, the review will be conducted every six months. Review once a year for a lifetime. A comprehensive examination was performed after 4 to 6 cycles of postoperative adjuvant chemotherapy in patients with combined chemotherapy after particle implantation. During the follow-up, tumor recurrence and metastasis were found and treated in time. Regular follow-up should last at least 5 years. 1. Consultation and physical examination asked whether the symptoms had changed before and after particle implantation, and whether there were any new symptoms, especially in respiration, cough, hemoptysis, chest pain, pronunciation, pain, limb movement, etc. Physical examination, especially the touch of superficial lymph nodes such as neck, supraclavicular and armpit, and the look, touch, knock and listen of the chest. 2. Tumor indicators CEA(carcinoembryonic antigen), SCC(squamous cell carcinoma associated antigen), Fer(ferritin), Ca125, Ca153, etc. 3. Chest CT was used to observe the changes in tumor size and whether there was intrapulmonary metastasis, mediastinal lymph node metastasis, or metastasis and invasion of pleura and ribs after particle implantation. 4. Functional metastases were observed by PET-CT or SET-CT. 5. Abdominal B-ultrasound should focus on liver, spleen, kidney, adrenal gland and abdominal lymph nodes. If in doubt, enhanced UPPER abdominal CT should be performed. 6. If there is localized bone pain, especially if it is aggravated or accompanied by tenderness, there is a possibility of bone metastasis. ECT bone scan should be performed first to understand the condition of bone marrow in the whole body, and MRI or CT should be performed on the positive parts. 7. If there is persistent headache, vomiting, unequal muscle strength of bilateral limbs, unstable walking, etc., the possibility of intracranial metastasis should be considered, and a neurologist should be consulted. 1. Efficacy evaluation 1 to 3 months after particle implantation is the best time for efficacy. If the tumor continues to grow at this time, it should be considered that the tumor is not sensitive to particle implantation or PD is small. The effect of the treatment is most significant when the radiation dose of the implanted particles reaches 80% six months after implantation. 2. Early diagnosis and treatment of metastatic cancer, so that it is effectively controlled. 3. Short-term and long-term complications were observed.
