Should I have a lumbar puncture for streptococcal infection?

(1) Do you consider central nervous system infection? (2) Are there symptoms of increased intracranial pressure due to structural damage? If meningitis is considered and there is no indication of increased intracranial pressure, a lumbar puncture should be performed immediately.

For acute trauma or manifestations of elevated intracranial pressure, CT should be selected as the diagnostic basis, while lumbar puncture is contraindicated. The patient's medical history is consistent with a central nervous system infection, and CT should be performed immediately; if negative, a lumbar puncture should be performed.

If such a patient cannot have a CT scan immediately, antibiotics should be started. Patients suspected of subarachnoid hemorrhage should have CT, but if negative, a lumbar puncture should be done to look for red blood cells or cerebrospinal fluid yellowing, as CT is not 100% sensitive for diagnosing subarachnoid hemorrhage.
