In traditional procedures, the needle is directly inserted without regard to resistance, which can cause "leakage and bruising" through tiny blood vessels. The extravascular blood degrades and fades over time, creating unsightly ecchymoses that are more uncomfortable for the patient than the treatment itself. Piercing other types of soft tissue, such as nerves, tendons, ligaments, and muscles, inevitably results in swelling and pain. The advantage of a blunt needle is that the blunt tip can slide away from these sensitive tissues instead of piercing them.

By "swinging the needle" the authors simply mean pushing the blunt needle in from the initial insertion point and very slowly and gently – so that you can directly feel close to the tissue we want to slip away. If a painful condition occurs, simply "twist" back a little and then reorient the blunt needle, advancing 1 or 2 mm in the new direction or plane until no resistance is felt, then continue advancing. According to the author's calculation, the "twisting" method can reduce bruising by 70% when using a blunt needle.
