Liver biopsy: ultrasound-guided percutaneous liver biopsy
Dec 10, 2021
(1) Select puncture needles: 16 G biopsy needle is recommended for liver tissue biopsy, 18 G biopsy needle is recommended for liver mass biopsy, and the length of liver tissue specimen should be at least 1.5 cm.
(2) Position: supine position or left decubitus position, the right side of the body close to the bed, the right arm lifted and bent behind the pillow.
(3) Disinfection and anesthesia: strictly aseptic operation. The operator wears a mask, hat and aseptic gloves, sterilizes the punctured local skin routinely, spreads aseptic towel, and infiltrates the skin, muscle and liver capsule of the punctured site layer by layer with 2% lidocaine.
(4) Percutaneous puncture: The patient was instructed to breathe calmly. The surgeon pierced the liver parenchyma with a puncture needle under the guidance of ultrasound and fired the needle after reaching the target position. Coaxial puncture technique: the coaxial needle matching the biopsy needle is punctured to the front edge of the target lesion, and the coaxial needle core is pulled out. The biopsy needle is biopsied in different directions through the coaxial outer sheath for multiple times. After sampling, the coaxial needle is pulled out under breath-holding state.
(5) Dressing: The puncture point was disinfected, sterile gauze was covered and fixed, and multiple abdomens were wrapped tightly around the rib and upper abdomen with pressure for at least 2 hours.
