Subdural hematomas and other lesions cause more severe symptoms than epidurals because the subdural is directly facing the brain tissue. The most common disease of an epidural is an epidural hematoma, which is a hematoma formed when the broken end of an epidural fracture punctures a blood vessel in the dura.
Subdural hematoma is usually caused by bleeding from the surface of brain tissue infiltrating into the subdural layer, often combined with cerebral contusion and laceration, so its clinical symptoms are significantly heavier than epidural.
