Development of the cutting spinal needle tip (2)

Oct 11, 2021

Bainbridge described a needle attached to a metal syringe in 1900. It has a small round hub, a short and sharp cutting bevel, and a stylet with a matching bevel (Figure 3). It is made of flexible metal. Buck is the next pioneer in needle design. He described his technique in 1907. Initially, he used a modified Bier needle, but when he realized the problems associated with the long bevel, he designed a hollowed-out needle tip to "ensure sharpness without extending the terminal opening." He said: "The purpose of this is that when puncturing, the entire open end of the needle should be located in the dura mater, without pushing the tip into the wound structure in the capsule. Obviously, if the open end of the needle only partially penetrates the hard The meninges and spinal fluid may flow freely enough through the needle, but when an analgesic compound is injected, part of it may enter the sac, while the other part may escape... the epidural space. According to this belief, I use the following device to deliver all injections beyond the actual point of the needle, regardless of whether the entire end opening is actually inside the capsule". Barker developed a blunt cannula to fit inside a Bier needle, protruding 1 mm from the needle tip. After puncturing the dura mater with a Bier needle, place it and connect it to the local anesthetic solution syringe. The blunt tip is designed to limit damage to the intradural structure. Later, without an unknown date, Buck designed a needle without an inner cannula, called a Buck needle (Figure 4). This 18G or 19G needle has a sharp mid-length bevel and a stylet with a matching bevel. This is a large and sturdy needle, which is significantly related to the incidence of PDPH. Early needles were made of "nickel-plated steel." They quickly change color and corrode. Buck suggested that the needle be made of hard nickel, which was a major development in needle manufacturing.
