What Is The Endoscopic Treatment Of Intestinal Polyp?

Nov 07, 2022

Endoscopic therapy is the most common way to remove intestinal polyps, especially colon polyps, and is most suitable for pedicled polyps. There are many endoscopic polypectomy methods, and different treatment methods should be selected according to the position, size, shape, pedicle or pedicle of polyps. In recent years, with the improvement of endoscopic therapy technology, there are more and more means of colonoscopy endoscopic resection, and the indications are expanding.

Generally speaking, for large polyps with pedici and pedici, the 2cm diameter can be directly inserted into the root of the polyp with a snare connected with high frequency electricity for one-time resection. Wide basal polyps larger than 2cm can be removed in sections or tied with nylon snare. Flat, pedicular-free polyps can be removed by endoscopic mucosal resection (EMR), in which epinephrine saline is injected into the submucosa of the base at different points. After the lesion is raised, namely, entrapment resection, which can not only prevent bleeding and perforation, but also achieve the purpose of treatment. In recent years, more and more doctors have adopted the technique of endoscopic submucosal dissection (ESD), which can even completely remove flat polyps of 5 ~ 10cm. For polyps smaller than 0.5cm, biopsy forceps are used to remove directly, and sometimes argon laser electrocoagulation is used to treat the polyps, which is safe and fast. Endoscopically removed polyps are usually withdrawn and sent for pathological examination.

Endoscopic polypectomy is generally safe, but there are also risks of complications. The most common complications include intestinal perforation. During endoscopic polypectomy, the entire intestinal wall may be cut through, leading to intestinal contents entering the abdominal cavity, resulting in pneumoperitoneum, abdominal pain, infection, etc.

Smaller piercings may be closed with an endoscopic metal clip, while larger piercings may require emergency surgery. Bleeding after polypectomy is also a common complication, and most of the bleeding can be stopped by endoscope to avoid salvage surgery.
