What Does PTHC Mean

Percutaneous transhepatic cholangiography (PTHC or PTC) or percutaneous cholangiography is used to visualize the anatomy of the biliary tract using radiographic techniques. Contrast material is injected into the bile ducts of the liver and X-rays are taken.

If endoscopic retrograde cholangiopancreatography (ERCP) fails, the biliary tree can be entered. The program was first reported in 1937 and became popular in 1952.

It is now performed primarily as a therapeutic technique. Less invasive methods of biliary tree imaging include transabdominal ultrasound, magnetic resonance biliopancreatic imaging, computed tomography, and endoscopic ultrasound. If the biliary system is obstructed, biliary drainage may be performed using PTC until a more permanent resolution of the obstruction (e.g. surgery) is performed.

In addition, self-expanding metal stents can be placed in malignant biliary strictures for palliative drainage. Percutaneous placement of metal scaffolds can be used when therapeutic ERCP is unsuccessful and the anatomy is altered to allow endoscopic drainage of part of the bile duct into the duodenum or liver to separate, allowing for more selective placement of metal scaffolds.

Biliary angiography during biliary drainage is called perioperative or primary cholangiography and, when performed later in the same drainage, secondary cholangiography.
