Post-OPU infection is rare - and that rarity is precisely what breeds complacency. When it occurs, the fallout is disproportionate: pelvic inflammatory episode, cycle cancellation, prolonged antibiotics, patient distress, and in litigious environments, legal exposure. The breaches are rarely dramatic. They are quiet: a probe cover applied with a trapped micro-bubble that later weeps gel into the field; a luer cap flipped open and rested on the drape "for a second"; a blister pack whose sterilization indicator strip was not glanced at; a collection tube procured from a vendor without MEA testing. Under Europe's MDR and tightening FDA scrutiny of reproductive consumables, the expectation has shifted: sterility is no longer assumed, it must be demonstrated and documentedper procedure. The needle is only one node in a sterile chain that includes cover, tubing, pot, dish, and human handling.
Principle of Safe Consumable Chain
From the metallic tip to the embryo dish, the path must be continuous, sterile, and embryo-compatible. The needle: 316L medical stainless, laser echo-marked, polished lumen, manufactured under ISO13485. The tubing: medical-grade silicone/PTFE, low-endotoxin, non-cytotoxic, MEA-compliant, kink-resistant. The luer hub: secure lock, no gaps. The probe cover: sterile, bubble-free, snug. The collection pot: sterile, pre-labeled, low dead space. The dish: pre-warmed, protein-supplemented. Flush between ovaries only with sterile warmed media. Discard the needle into sharps in one motion - no "set it down." Scan the QR into the patient's electronic record: lot, expiry, operator. At month's end, review near-miss logs: a torn cover, a kinked tube, a delayed-expiry catch - these are leading indicators, not noise. Procurement often scores needles by unit price. A mature total-cost model factors: oocyte loss from lumen clogging, blockage-related time loss, anesthesia top-up rate, junior training time on poor-handle needles, recall risk from non-ISO vendors, and documentation labor. One director's blunt math: "Saving €0.80 on a needle can cost a full ICSI cycle and a complaint." Some groups now require vendors to supply MEA certificates and batch endotoxin reports, not just a CE mark. The cheapest line item is rarely the lowest total cost of ownership.
