The nodules in the thyroid are not necessarily the same as the nodules in the lungs. The problem is we don't know if there are lymph node metastases in the mediastinum. If it is a tumor and there are mediastinal lymph node metastases, it may not be surgically resolved. But if you have lymph node metastases in the mediastinum you can do a mediastinoscopy and get a pathology to confirm the diagnosis. If there is no alternative, puncture treatment is the only option. You can't just ignore it because you're worried about the needle transfer. The probability of the transfer is not as high as you think.
